SAAZ Referee Send Off Report (Online version - GotSport)

• This form to be used for SAAZ managed leagues NPL and AZDL.
• This form is to be completed by referees only and to be used when players, coaches and team officials that receive a red card and/or are sent off.

Report Referee Harrassment

• This form can be used to report if you witness a referee being harassed or abused.
• This report may be submitted by any person for observation of referees being mistreated or abused by any participant of a match.  This may include but is not limited to players, coaches, directors of coaching, other club representatives, spectators, and parents.

Coach Evaluation Form for Referees (Coming Soon)

• This form is to be completed by coaches, team officials, or club officials to both recorgnize great referee performances, as well as provide feedback to help referees improve.
• Video may be used as an attachment
• Submissions are reviewed by the League and passed on to the referee assignor and State Referee Administrator.

• Q:  What should I do if I have an issue with a referee?
• A:  Speak to your coach, club official, or contact the league.  Do not yell at the Referee during the match.
